Send a request into the Fail Modes support workflow. When email support is configured, it is also sent to the dedicated inbox; otherwise it is recorded in the operator queue. We only show success after the request is accepted for operator review.
Purchase and billing help
The free diagnosis helps identify the evidence gap or unsupported assumption behind a product, support case, operational incident, or workflow. It is free during Public Alpha and useful on its own. Paid analysis is not currently purchasable.
Checkout is gated, so no payment can be made while this safeguard is active. Features may change as the alpha develops.
If you were ever charged but did not receive the promised analysis, review the refund policy and include your diagnostic ID when contacting support.
Refunds
Paid features are currently disabled: no purchases can be made, so no refunds can occur right now. The policy below applies only if paid analysis becomes available.
Completed digital reports and action plans are non-refundable. If payment is taken but the paid artifact is not delivered after the service retry limit, the original payment is automatically refunded. An unfavorable diagnosis or disagreement with the result is not a refund trigger. See the full refund policy in the Terms of Service.
Privacy and terms
Read the Privacy Policy to learn what data is collected, how diagnostic data is used, and how to request deletion. The Terms of Service explain the service limitations and refund policy.
Known limitations
- Results identify failure mechanisms from supplied evidence. They are attestations for evaluation, not authorization; reported claims do not become verified facts because the diagnostic is confident.
- The diagnosis relies on the publicly visible website content and information you provide.
- Your saved result link is the primary access path. Results also offer a JSON download. Email is optional secondary delivery; a free PDF download is not currently offered.
- Support is available through the request form above; a public email address is not currently exposed.
Status / incident note
Current status: Public Alpha — free diagnosis available. Paid checkout is gated and not currently purchasable. No paid fulfillment is being offered tonight.
Why this exists
Fail Modes is an evidence-first failure diagnosis layer built around a shared discipline: intended outcome → observed record → bounded diagnosis. It preserves uncertainty and ties recommendations to observed facts. Fail Modes does not send your diagnostic to another service, and no outside evaluation runs automatically.
Evidence rule
A configured step is not proof that it ran. A successful request is not proof of a completed outcome. A listed prospect is not proof of a sale. A diagnosis does not authorize changes.
